Interpersonal Process Recording Guidelines and Template Fall 2024 Description: An Interpersonal Process Recording is the formal record that documents, in detail, the verbal and non-verbal interaction between the student and another individual. It provides an opportunity for students to perform an in-depth analysis of their own thoughts, behaviors, feelings and words, as well as examining more closely, the flow of communication between themselves and another person. The Process Recording serves to help students identify places in the interaction where key cues were missed or misinterpreted and their own responses were lacking. In the space provided below document a segment of an interaction, 6 – 7 exchanges (an exchange constitutes an utterance from each participant in the interaction). For example, if the patient states “I am feeling scared about my family meeting this afternoon, I know my wife is going to say she is not ready for me to come home, that she can’t deal with having me there.” and the nurse responds “You’re feeling worried about the meeting this afternoon, concerned about your wife’s response to your discharge next week.” An exchange includes both a patient and a student utterance. As soon as you complete your interaction take some time to make some detailed notes so that when you write up your interaction for the assignment you will have the necessary information. Documentation of Interaction Analysis: Students are expected to utilize texts or handouts on therapeutic & non-therapeutic communication, defense mechanisms, etc. to document their analyses. References: Student will be expected to use 2 references, 1 from your textbook and another from a scholarly article. Scholarly articles can be found in peer-reviewed journals. It is written by experts in an academic or professional field (i.e.: Journal of Communication in Healthcare). List all sources used in preparing your Process Recording using APA 6th ed. format. 1 Interpersonal Process Recording Template Preliminary Data Sheet 2 Please provide the following demographics: The individual’s Initial of First or Last Name: (DO NOT record full first or last name) (Consider this a very confidential documents to be kept in a very safe and secure place at all times) Individual’s Age: 53 Gender/Sexual identity: Female Race/Ethnicity:Asian Religion/Spiritual Practice: Muslim Religion/Spiritual Practice: Muslim Marital Status: Married: X , Living common law___, Separated___, Divorced___, Widowed____, Single_______ Please provide the following information to set the context for the interaction: 1. Context of interaction (where and when it is taking place, whether you have worked with this patient on a previous day(s), whether you have interacted with the patient previously on the day of the interaction you are recording): Interaction with PT has occurred over several different clinical days in several different locations, like the Med window during distribution of medication and the common space during free time. Interactions lasted as long as PT wanted to interact for. Interactions were vaseline interactions in order to build trust and comfort within the nurse-client relationship and before asking more personal question in order to allow PT to feel comfortable with the questions and/or irritable 2. Goals for this interaction and planned actions to achieve your goals: My 1st goal is to build trust within the pt-nurse relationship My 2nd goal is to have a deeper conversation with the PT in order to understand the PT more, their mental status, the reason they are here and what their goals are for their future 3 Student Verbal/Nonverbal Patient Verbal/Nonverbal Analysis “Good Morning” followed with a simple introduction Pt replied with a “Good Morning” simple smile and nod Client was calm and pleasant during interaction out of politeness but was not interested to further continue conversation Giving Med to PT while smiling Pt replied with a “Thank you” while taking meds and “Do you know when I’m leaving?” Pt shows interest in their recovery as they are taking their med as ordered without resistance and is asking about their future outside of the unit Instructor introduced self to PT, I also did a simple introduction PT was sleeping, and only said simple Hellos I need to be more confident in interacting with PTs and take control in the conversation in order to build a relationship with PT Instructor and I were trying to form a conversation with PT PT did simple replies and kept turning around to go back to sleep PT didn’t want to have conversation with us, as indicated by her nonverbal actions but didn’t want to be rude as indicated by her verbal actions Pt came up to me to ask for a cup for water As PT and I had a few interaction and has seen me around the unit this could show that she has some sense of trust towards me as some of her symptoms due to her condition include paranoia. This interaction could also be an indications that the meds are working for her and her symptoms 4 Adapted by DFG -Fall2017, from https://canvas.uw.edu/courses/1024792/assignments/3076348 Student Verbal/Nonverbal Patient Verbal/Nonverbal Analysis I asked Pt how they were feeling Pt replied with “Good” and spoke briefly about how they been feeling and what they have been doing I feel like im gaining some confidence in speaking with the PT and building a relationship with PT. PT is also opening up to me and is feeling conformable to tell me how they are feeling At the end of our conversation pt Thanked me for asking and talking to them This interaction could show that we are building a healthy relationship between us as nurse-pt as she is starting to feel comfortable talking to me and conversations are starting to get longer. Pt could also be happy that we are taking even if its a small and simple conversations as shown through her comment of thanking me As I was leaving the unit, PT came up to the nursing station to say Goodbye This could be an indication that the PT trust and likes me. It could also show that the medications are helping her and her mental stability as she is no longer showing signs of aggression. 5 Reference Halter, M. J. (2022a).
