This is assessment 1: Case Management Plan HLTH3005 INTERPROFESSIONAL HEALTH PRACTICE ASSESSMENT BRIEF Assessment Summary Scenario of Felicity interview: Felicity who is 26 weeks pregnant of her first baby which she said super exciting. Felicity stated it was long time for her to decide whether to have a baby or not. Her husband who she mentioned is lovely Ross who has cerebral palsy which is why took them long time to decide whether right thing for them to have a baby . She said that when she fell pregnant she was super excited and also her husband. But just recently those excitement and the buzz about the baby has really turn to huge anxiety and stated “to be honest” she quite scared about whats going to happen when the baby is born. Falicity stated “to be honest I’m scared about having a baby because of that”. Falicity further stated that Ross got cerebral plasy and he uses a wheelchair most of the time. He can walk for short period of time without the wheelchair but he has got a really unsteady gait and they are worried that he is going to fall. Ross also got a left upper arm weakness so felicity states that she is worried about things like changing the baby, or holding or giving the baby a bath. Felicity stated she is really anxious of how Ross going to cope doing it. she stated that she knows she is going to be a complete mother hand when the baby comes along, it just she is worried and scared to leave the baby with Ross on his own in case something happens and so, super scared about that. Falicity states that Ross and her always talks about everything together but she is just worried now about she don’t want him to think about she doubting his ability as a father because she knows that she not and and know that they are both a team and on it together. But she just don’t want him to feel bad and she knows that he feels bad already so she is super concerned about how they are going to cope and how is he going to cope whether the baby is going to be safe. The case management plan is designed to engage students and assist in the application of knowledge of professional roles and responsibilities, relationships and collaboration to a complex patient scenario. Through this assessment, you will meet unit learning outcomes 1, 2 and 4. The assessment will support your preparation for working interprofessionally as a registered practitioner and assist you in working towards assessment 2. Task Description This assessment requires you to work individually to prepare a case management plan and a written piece for one patient scenario. The case scenario alongside the case management plan template. After completing the case management plan, you will need to provide a written piece of no more than 1,000 words (+/-10%), which clearly articulates why you have made the decisions that you have. These decisions should be supported with discipline-based evidence, as well as interprofessional evidence. To successfully pass this assessment, you will need to demonstrate the following: • A completed CMP template including identified issues of concern, goals of intervention, recommended interventions, identified disciplines and priorities of care. • Knowledge of the roles and priorities of care that each interprofessional team member will play in your patient scenario. • Knowledge of challenges that may arise when working in potentially large and diverse teams and resolution strategies (as applied to the patient scenario and your choices). • Ability to apply discipline based and interprofessional evidence to support the content in your CMP. • Inclusion of reputable academic sources published within the last seven years. Websites other than reputable journals and .gov sites are not considered appropriate. A reference list must be provided. Some points you may like to consider include: 1. Why have you included the healthcare practitioners that you have in the care of this patient? What can they bring to the team? Could another healthcare practitioner deliver this care instead of them? Your written piece should include the integration of discipline-specific and interprofessional evidence. • Reputable academic sources should be included and citation of these sources should follow APA7 style.
