respond to this: The condition I chose for this assignment is hypothyroidism. I chose this condition because hypothyroidism is a common disorder seen in the primary care setting with adults, which is who I will be working with as an adult gerontology primary care nurse practitioner. Hypothyroidism is the insufficient production of the T3 and T4 thyroid hormones. It is most often caused by autoimmune disease, thyroidectomy, or iodine deficiencies. Signs and symptoms of hypothyroidism include weight gain, fatigue, and cold intolerance. Per the clinical practice guidelines (CPG), levothyroxine is recommended as the primary treatment, and dosing is based on age, weight, and TSH levels (Garber et al., 2012). When considering different populations, weight-based dosing and adjustments will be necessary for pediatric patients and pregnant women may need increased doses to support fetal growth. The clinical practice guidelines are important to help clinicians provide evidence based care but there are some limitations including little guidance on combination therapy and subclinical hypothyroidism (Guyatt et al., 2006)
