Palliative care is mainly focused on managing symptoms and improving quality of life for those patients with serious illnesses and is available to all ages. Palliative care can be stated as early as the patient is diagnosed with the terminal illness; this can be provided in a hospital, nursing home outpatient clinic or home. Hospice care is a specialized form of palliative care that focuses on the care, comfort and quality of life when a patient is nearing the end of life, where curative treatment is no longer the goal. Hospice care usually begins when the patient is no longer responding to medical treatment that either cures or slows the disease progression. As a nurse who is supporting the patient’s wishes regarding end-of-life care, it is our duty to make them feel as comfortable as possible. This can be done by actively listening to the patients, facilitating open communication, managing symptoms, advocating for their choices and providing emotional support. An advanced directive is a form where an individual appoints a person to make health care decisions for them when they are incapable of doing so themselves; this can be used in any healthcare setting. A POLST is a medical order from a provider that provides instructions for the patient for medical staff in emergency situations. In this form, the patient must specific what they want and what they don’t want. In situations where EMS is involved, the emergency medical technicians must honor the POLST orders, as opposed to advanced directives, EMS must provide necessary care to stabilize patient for transfer to hospital.
