She has Past Psych history of Alzheimer’s, Dementia, Delirium due to known physiological condition. Current meds include Aricept 10mg daily at HS for dementia. Patient is alert with some confusion. She was in the room with her husband during assessment. Husband said her mood has been so down since she went to the hospital. She just had a right hip and wrist sugery on 2/20/ 2025 at virginia Mason Hospital . She verbalized being sad from not being able to use her wrist and hip again. Patient was encouraged to continue working with therapy for strengthening She vebalised missing the things she used to do at the assisted living were she has been living for two years (she loves playing games at the facility and husband always comes to take her for a ride). She said right now she is just laying here and waiting for help. She now needs assistance with feeding due to the recent wrist surgery. Speech tone was normal, speech not coherent, some confusion.
