CHIEF COMPLAINT: “My cough is getting worse, and now it hurts.” HPI: Patient is a male who presents with a 2-day history of left-sided pleuritic chest pain, dyspnea, orthopea, intermittent fever and chills, and a cough productive of yellow sputum, which became brown yesterday. The pleuritic chest pain radiates to the proximal LUE during coughing. He experienced no nausea but did have one episode of emesis yesterday. He also states that he had one episode of rhinorrhea this morning and had a sore throat, which has now resolved. PMH: Asthma since early childhood, which is under good control despite the fact that he ran out of his albuterol inhaler and has not been using it for some time. FH: Positive for HTN and negative for CAD, asthma, DM, and cancer SH: Patient denies tobacco or drug use. Denies having received a blood transfusion. Reports occasional alcohol use. ROS: In addition to the findings reported in the HPI, he is currently experiencing a mild headache and reports intermittent headaches, occasional dysuria, and decreased appetite. He reports normal BMs without blood and noted no blood in his emesis. He reports no dysphasia, weakness, or any known infectious contacts.
